Families and carers are often central to supporting older people. We can help families understand available services, referral pathways, social groups, meals, transport, and respite options
HMC supports older people who are over 65 years old, including people from culturally and linguistically diverse backgrounds, migrants, refugees, and established multicultural communities
Some services may require assessment through My Aged Care or another referral pathway. HMC can help explain the process and guide you to the right next step
